A 22-year-old Nepali male patient presented with fever and weight loss of 1 month duration. His medical history was unremarkable. On examination, he was febrile (temperature 39°C) with anterior chest wall swelling above the right nipple. Chest X-ray is shown in [Figure 1] as well as computed tomography (CT)-scan chest [Figure 2]a and CT-scan abdomen [Figure 2]b.{Figure 1}{Figure 2}

There is a right anterior chest wall cold abscess with posterior extension with multiple lung opacities (see arrows in [Figure 1] and [Figure 2]a). There is also left paraspinal multiple psoas abscesses (see arrow in [Figure 2]b). These findings consistent with tuberculosis.
